Health Insurance

Enrollment Requirement:  The California State University (CSU) system requires all non-immigrant international students to be enrolled in adequate insurance coverage for health care, medical evacuation, and repatriation, beginning with the start of your program and maintained at all times thereafter while enrolled at a CSU campus in F1 or J1 visa status. 

Enrollment Process:  All international students will be automatically enrolled in a pre-approved plan and will pay their health insurance fees through their Portal account each semester.  Your insurance payment will be due at the same time as your registration fee payment.  Students may also elect to enroll eligible dependents (spouse or children).  Please email international@humboldt.edu if you would like to enroll dependents in the health insurance plan.

Enrollment Dates & Estimated Cost:

Fall policy dates :  August 11, 2024 to January 7, 2025 - $773.00

Spring/Summer policy dates:  January 8, 2025 to August 10, 2025 - $1,107.00 - if you are not graduating at the end of a spring term, you will automatically be enrolled in the spring/summer coverage plan.

Note - Will you be traveling outside the U.S. between May 31, 2025 and August 10, 2025?  Email international@humboldt.edu to request an exemption from summer health insurance enrollment.  Proof of travel will be required (flight itinerary).  The deadline to request an exemption is May 1st.

Exemptions:  The only students who qualify for an exemption from enrollment in the pre-approved Cal Poly Humboldt plan are:

  • Government sponsored students covered by a U.S. policy provided by their sponsor
  • Dependents of a parent or spouse employed in the U.S. who provide evidence they are covered by the U.S. employee-based policy and that policy is accepted at local providers.

Approved Plan:  Cal Poly Humboldt has arranged coverage with a pre-approved policy via United Healthcare which meets the CSU minimum requirements.  The Dean of the College of Extended Education & Global Engagement with input from the Global Engagement Team has carefully selected an insurance plan that provides a policy designed to specifically meet the needs of international students.  This policy has low out-of-pocket expenses with local doctors, including Redwood Urgent Care Center and emergency services at Mad River and St. Joseph/Providence Hospitals. 

Medical Care:  Students are encouraged to visit the campus Student Health Center (SHC) for all routine and non life-threatening medical needs.  A student may be referred by the SHC to a health care practitioner in the local community, but that referral may or may not be to a Preferred Care Provider.  Always check with the provider coverage by visiting United Healthcare website or calling United Healthcare customer service line (1-800-767-0700).  You can also call the customer service line for any questions related to benefit coverage or claims.  

For weekend care needs (non-life threatening) we encourage students to visit the Redwood Urgent Care facility in Eureka, open 7 days a week on a walk-in basis.  Redwood Urgent Care does take United Healthcare insurance.  You can visit their website for more information, HERE.

Hospital Emergency Rooms and walk-in centers should be reserved for life-threatening or after-hour medical emergencies.  Emergency room visits could cost you a lot more money because the doctor on site may be out-of-network.

Insurance ID Card:  When your ID card is ready to print, you will receive an email from United Healthcare (UHCSR) with a link to your card.  Complete details about the benefits covered by the United Healthcare policy (and limitations and exclusions) and instructions on how to obtain your medical ID card will be emailed to you.  If you do not receive the email you can find the information on their website: here after your enrollment date.  You can also obtain your ID card through the United Healthcare Student Resources mobile app available through the Apple App store or Google Play by searching UHCSR.  

Keep your ID card with you at all times and present it whenever you receive medical treatment.

Definitions

Emergency Room (ER): Part of a hospital that's open at all hours, every day, to treat severe and life-threatening conditions. When you believe that your condition may permanently endanger your health or lead to your death if not treated promptly, use of the ER is appropriate.

Preferred Care Provider: Any hospital, physician, ambulatory surgical center, durable equipment outlet, clinical laboratory, skilled nursing facility, or diagnostic imagining services facility that has an agreement with United Healthcare to accept the negotiated charges for covered services in effect at the time services are provided to the patient. Use of a Preferred Care Provider will save the patient money when compared to the same services of a non-Preferred Care Provider.
